S8 - Effect of Implementation of a 2:1 Clinical to Simulation Hour Ratio on Pre-licensure Nursing Student Outcomes

Abstract : Simulation has become an essential element of nursing education, yet limited research exists on clinical-to-simulation hour ratio. This pilot study sought to explore differences in cohort outcomes among pre-licensure nursing students after implementation of a 2:1 clinical to simulation hour ratio compared to a 1:1 clinical ratio. Using a two-group comparative design and retrospective secondary data analysis, the study examined traditional Bachelor of Science in Nursing students at a midwestern university's nursing extension program. The study compared critical thinking levels, entry-level competency, and NCLEX-RN© pass rates between two groups: those with 1:1 clinical-to-simulation experiences (Group 1) and those with 2:1 experience (Group 2). The Iowa Model of Evidence-Based Practice to Promote Excellence in Health Care was used as the theoretical model for this study. This model guides decision-making at the organizational level with decision points and feedback loops throughout, emphasizing a pilot project prior to implementing program wide change. Results showed no significant differences between the two groups on mean critical thinking, comprehensive predictor scores, or NCLEX-RN(c) success rates. Group membership was not associated with success on the NCLEX exam. These findings suggest that implementing a 2:1 clinical to simulation hour ratio did not significantly affect student outcomes in this sample.
