F1 - Cultural Responsiveness and Inclusion Evaluation Tool for Health-Related Simulation

Abstract : Simulation is an evidence-based teaching method in healthcare, yet few studies have evaluated learner perceptions of its cultural responsiveness and inclusion—an important ethical consideration for training a diverse workforce. Supported by Cultural Competence Theory, which emphasizes understanding and respecting diverse cultural backgrounds, we aimed to evaluate how well simulation scenarios meet the needs of varied populations and guide the development of culturally responsive curricula.

Our international team of nurse researchers collaborated to design and test the Cultural Responsiveness and Inclusion Evaluation Tool for Health-Related Simulation. Initially, we created 65 questions organized by subscale in accordance with INACSL Healthcare Simulation Standards of Best Practices (INACSL, 2021). To establish content validity, we distributed the tool to simulation experts worldwide using purposive and snowball sampling.

The tool was ultimately tested among nursing students in India participating in simulations aimed at reducing infant mortality. We received responses from 32 simulation experts (84.4% female, 15.6% male) from six countries (India, USA, Thailand, Saudi Arabia, Pakistan, and Chile). Based on survey feedback, we eliminated 12 questions and edited six.

When tested in India, the tool generated 919 responses from students involved in simulations focused on shoulder dystocia, congenital cardiac conditions, and fetal distress. The internal consistency reliabilities of the tool’s subscales showed Cronbach’s alpha values ranging from 0.766 to 0.950, demonstrating good reliability. This research underscores the need for culturally responsive training in healthcare simulation.
