(28) Small Talk: Big Impact on Caring for Families with Fetal Demise

Abstract : With the uncertainty of rapidly changing regulations/legislation/care options, APRNs must be prepared to care for families seeking access to a stillbirth induction and delivery. The simulation design uses objectives founded on the American College of Nurse-Midwives Core Competencies and National Organization of Nurse Practitioner Faculty Standards. The simulation’s learning objectives also reflect and demonstrate expected AACN Competencies and Sub-competencies for entry, and advanced levels from Person-Centered Care, Interprofessional Partnerships and Professionalism Domains.
One to two pre-licensure and graduate nursing students fulfill primary nurse and provider roles. Dedicated pre-brief includes content and bereavement self-assessment to precede the active encounters, a four-hour scheduled block accommodates up to twelve to twenty-four students followed by a reflective learning debriefing model. Team members include a simulationist, faculty content experts, staff, and technicians. A standardized patient plays a family member with scripted cues that help learners achieve outcomes. Finally, the standardized patient holds a preterm mannequin to create fidelity of a third-trimester loss. Interprofessional aspects may include chaplain support, physician consultation, financial counseling, social work assessment, case-management discharge resources and follow-up with primary care. Evaluation rubrics reveal leveled performance based indicators that demonstrate what learners can do with what they know. An adaptation of a Soft Skill’s Rubric (Yong et al’, 2024) is discussed.
